Care built around the whole person

One practice. More ways to feel supported.

Chronic care management — structured follow-up between visits for patients managing multiple conditions.

Infectious disease support — evaluation, treatment follow-through, and monitoring alongside your other care.

Why whole-person care

Health rarely arrives in one category at a time.

Blood pressure is affected by sleep. Sleep is affected by anxiety. Anxiety is affected by pain, by transportation, by whether anyone followed up after the last hospital stay. When those pieces are treated separately, patients end up repeating themselves and falling through the gaps.

A-Nu Health keeps medical and mental health care under one care team, so the plan accounts for the whole picture — and someone is responsible for the follow-through.

  1. We look at everything together

Medical history, medications, mood, mobility, and daily circumstances in one evaluation.

  2. We build a plan you can follow

Realistic steps, explained in plain language, adjusted to the week you actually have.

  3. We stay with it

Follow-up visits, labs, medication changes, and coordination with the people already involved in your care.
