How we handle information on this site
Information you choose to send
Contact forms, callback requests, and screening questionnaires on this site collect only what you type into them. We ask that you do not include detailed medical records, sensitive identifiers, or urgent clinical information in website forms.
How submissions are used
Submissions are reviewed by A-Nu Health staff so we can respond to your request, schedule an appointment, or follow up on a questionnaire. Website submissions are not monitored continuously and should never be used for emergencies or crises.
Third-party tools
Appointment scheduling and secure referral submission are handled on external platforms operated by other companies. When you follow those links, their own terms and privacy practices apply.
Your rights as a patient
Patients of the practice receive a separate notice describing how protected health information is used and disclosed, along with the rights available to them. To request a copy or ask a question about your record, contact the office directly.

A-Nu Health does not exclude, deny benefits to, or otherwise discriminate against any person on the basis of race, color, national origin, ancestry, age, disability, sex, gender identity, sexual orientation, religion, or veteran status.
